A comprehensive follow-up to assess your progress with your care plan, quality of life, recent medical appointments, new test results, and life changes. We’ll address questions related to medication, therapies, symptoms, and self-management, adjusting your care plan as needed to reflect changes in your condition. If applicable, you will receive an amended care plan or resource materials following your appointment.
