Service Description

An in-depth review of your life and medical history. We will dive into diagnoses, medications, testing, past treatments, head-to-toe symptoms, specialists involved in your care, stressors, social support, current abilities, quality of life, and goals of care. After the session, you will receive a personalized care plan with priority nursing diagnoses, actionable steps, and goals to begin improving your health immediately. You will also receive individualized resources, including tips for your situation, habit trackers, and symptom tracking tools. In following sessions, we will continuously review your status and progress, and adjust your care plan and goals as needed.
